Step 1:
Complete the Physician Update Form and your provider Cover Letter and any other items listed on the Credentialing Checklist MD / APP if you currently have them available to provide for processing.
Step 2:
Complete membership participation agreements, enrollment and supporting documents via DocuSign. To begin the DocuSign process, click the region (CT/MASS) below the aligns with your provider:
Step 3:
Please ensure all documents from the Credentialing Checklist are available/complete for processing a credentialing file. A member of the credentialing team will be in touch if additional information is needed for processing.
* Please ensure CAQH is current and re-attested to within the last 90 days or sooner.
** All the above is required for the provider to be presented to the SoNE HEALTH Credentialing Committee.

Changes and Terms for a Current Provider
Step 1:
Complete the Physician Update Form and your provider Term or Change letter to upload in the area located at the bottom of the form.
*When submitting a Change notification please ensure the change is also reflected in CAQH and re-attested.
