The focus in our general social work program comprises of outreach and coordination of community members and patients with Social Influencer of Health (SIOH) needs. An assessment of a person’s SIOH needs is performed with needs and supports then coordinated. Services include coordination of financial assistance with utility, rental, medical bills, housing assistance including completing applications, and collaborating other social services like town or senior services including medical transportation. Services also include screening for safety concerns and social isolation toward coordinating appropriate services, including caregiver support.
