The licensed clinical social workers in our Behavioral Health care management program focus on supporting individuals with behavioral health needs. Their work begins with building trust and rapport with individuals diagnosed with a variety of mental health conditions, including depression, anxiety, and substance abuse, toward empowering them to manage their signs and symptoms, to maintain behavioral health appointments, and to offer additional resources in the community as needed. Depression and patient safety screenings are performed. Collaboration with primary care and mental health providers in the community are integral in meeting health and wellness goals as well as recovery support.
