Quality is an important piece of our overall value-based performance as it reflects the standard of care we provide to our patients. Quality performance is measured through preventive care screenings, follow up care for chronic conditions, medication adherence, immunization status, and patient satisfaction surveys.
To proactively manage the quality of care provided throughout our network, we leverage data registries to identify patients who need follow up, and a Quality Performance team works with providers on optimizing workflows, pre-visit planning, and providing ongoing education on the quality measure requirements.
