Southern New England Healthcare (SoNE HEALTH) Enables Behavioral Health Support at the Primary Care Level in New Collaboration with Concert Health

strategic collaboration brings nearly 2,000 primary and specialty care physicians support and delivers high-quality, behavioral health services to patients across Southern New England region

HARTFORD, Conn. & SAN DIEGO, Calif.–(BUSINESS WIRE)–Southern New England Healthcare (SoNE HEALTH), the region’s premier value-based healthcare organization, together with Concert Health, a leading behavioral health medical group, have entered into a strategic collaboration to integrate treatment for behavioral health conditions in the primary care setting. Together, SoNE HEALTH and Concert Health will deliver Collaborative Care, an evidence-based model that is proven to identify and significantly decrease symptoms of behavioral health conditions within the primary care setting.

The combination of our population health approach with Concert’s systemic model to identify and treat behavioral and mental health conditions allows us to break down treatment barriers for thousands of patients who would otherwise not have access.

“We’re committed to continuing to identify sustainable solutions in response to the ever-evolving healthcare landscape,” said Sarika Aggarwal, MD, MHCM Chief Population Health Officer at SoNE HEALTH. “There’s a critical need for behavioral health services right now. The combination of our population health approach with Concert’s systemic model to identify and treat behavioral and mental health conditions allows us to break down treatment barriers for thousands of patients who would otherwise not have access.”

A growing shortage of specialty mental health providers has resulted in limited access to high-quality and affordable mental health treatment, often leaving patients’ mental health disorders to go under-diagnosed, and undertreated.

The collaboration offers SoNE HEALTH’s primary care practitioners immediate access to Concert Health’s team of 130 Behavioral Care Managers and psychiatrists to connect patients to care within 24 hours from the privacy of their homes, develop behavioral health care plans, review symptom reduction, and make any necessary adjustments.

“We’re proud to support SoNE HEALTH’s mission to continually improve the quality of care and outcomes for their patients,” said Spencer Hutchins, co-founder and CEO of Concert Health. “Since 2017 we’ve been partnering with organizations that want to raise the standard of care for patients. As we continue to team up with more providers throughout Connecticut, we are committed to bringing evidence-based treatment to thousands of patients during a time where the need is reaching record heights.”

Concert Health’s flexible, patient-centered approach places SoNE HEALTH’s primary and specialty care physicians at the center of all physical and behavioral aspects of patient care—enabling them to take a population health approach to behavioral health. As described by Lisa Trumble, President and CEO of SoNE HEALTH, “Our partnership with Concert Health is the next step towards empowering our clinicians to address all aspects of their patients’ needs and health—not just the traditional, physical ones. There is a proven connection between mental and physical health and by integrating treatment with outcome measures for depression and anxiety, we’re not only improving care outcomes, but we’re bringing patients much needed, high-quality treatment in an environment they already trust.”
